A Trichology-Based Approach to Telogen Effluvium at Freedom Clinic Toronto
At Freedom Clinic Toronto, we do not look at excessive shedding as a single symptom in isolation. Telogen effluvium may be temporary, but the more important question is:
Why did the shedding start, what is happening in the follicles now, and is another hair-loss condition occurring at the same time?
Our trichology approach is designed to answer those questions.

1. Microscopic Hair and Scalp Analysis
A detailed microscopic examination can provide information that is difficult to determine simply by looking at the hair in a mirror.
During a trichology consultation, we assess different areas of the scalp and look for changes such as:
- Increased numbers of shedding telogen hairs
- Differences in hair-shaft diameter
- Miniaturization of follicles
- Shorter or weaker regrowing hairs
- Changes in overall follicular density
- Extended follicular dormancy
- Scalp inflammation
- Excess oil and follicular residue
- Scaling or flaking
- Signs that may suggest another scalp or hair-loss disorder
This is particularly important because telogen effluvium and androgenetic alopecia can occur together.
A patient may initially believe that all of the thinning is related to sudden shedding, while microscopic examination may reveal underlying follicular miniaturization consistent with male or female pattern hair loss.
Identifying that difference can significantly change the treatment plan.
2. Establishing Your Normal Hair-Shedding Pattern
Not everyone who sees hair in the shower has abnormal shedding.
We assess the pattern, duration, and severity of shedding rather than relying only on the number of hairs a patient notices at home.
We may consider:
- When the shedding began
- Whether it started suddenly or gradually
- Which areas of the scalp are shedding most
- Whether terminal and finer hairs are both being lost
- Whether shedding is still active
- Whether new growth is visible
- Whether overall density is recovering
Monitoring these factors over time can be much more useful than repeatedly counting hairs in the shower drain.
3. Looking Back Two to Four Months for Possible Triggers
One of the most important parts of a telogen effluvium consultation is taking a detailed history.
Because telogen effluvium is often delayed, we usually look several months before the shedding began.
Possible triggers can include:
- Illness or fever
- COVID-19 or other infections
- Surgery
- Hospitalization
- Significant emotional stress
- Major life changes
- Rapid weight loss
- Restrictive dieting
- Changes in protein or calorie intake
- New medications
- Discontinuation of medications
- Hormonal changes
- Pregnancy or postpartum changes
- Menopause
- Nutritional deficiencies
- Thyroid abnormalities
Sometimes the trigger is obvious. In other cases, several smaller factors may have contributed.
4. Blood Work When Appropriate
Hair follicles are extremely metabolically active. Nutritional, hormonal and systemic changes can influence the normal hair-growth cycle.
When the history suggests that further investigation is appropriate, we may recommend that a physician order blood work.
Depending on the individual, this may include investigation of:
- CBC
- Ferritin and iron status
- Vitamin D
- Vitamin B12
- Zinc
- Thyroid function
- Testosterone and androgen-related hormones when indicated
- Other nutritional or metabolic markers based on the patient’s history
We generally prefer identifying an actual deficiency before recommending high-dose supplementation.
Taking excessive amounts of iron, zinc or other nutrients when they are not required is not necessarily beneficial for the hair and may create other health concerns.
5. Treating the Scalp Environment
Telogen effluvium originates within the hair cycle, but scalp health should not be ignored.
Many patients experiencing excessive shedding also present with:
- Seborrheic dermatitis
- Excess oil
- Follicular buildup
- Itching
- Scaling
- Redness
- Scalp inflammation
These conditions are not necessarily the primary cause of telogen effluvium, but maintaining a healthy scalp environment is an important part of supporting normal hair growth.
At Freedom Clinic Toronto, scalp treatment may include a deep-cleansing Residue on Scalp Treatment (ROST) when significant oil, scaling or follicular buildup is present.
The purpose is to thoroughly cleanse the scalp and follicular openings and improve the scalp environment before beginning an appropriate home-care program.
6. Appropriate Shampooing — Even During Heavy Shedding
One of the most common behaviours we see in people experiencing sudden hair loss is reducing how often they shampoo.
Patients are understandably frightened when they see a large amount of hair coming out during washing.
However, avoiding shampooing does not stop telogen effluvium.
Those hairs have already entered the shedding phase.
When shampooing is delayed for several days, the loose hairs simply accumulate and may all be released during the next wash, making the shedding look even more severe.
For patients with excessive oil, dandruff or seborrheic dermatitis, we may recommend more frequent shampooing rather than less.
Depending on scalp findings, we may recommend the Freedom Clinic 2% Zinc Pyrithione Medicated Shampoo several times per week or, in some cases, more frequently during periods of active scalp inflammation.
The shampoo should be massaged thoroughly through the scalp rather than applied only to the hair.
7. Supporting the Hair-Growth Cycle
When telogen effluvium is confirmed and the follicles remain healthy, time is often one of the most important components of recovery.
New hair does not immediately replace the hair that has been shed.
The follicles must first re-enter the growth phase, and visible improvement in density can take months.
Our goal is therefore not simply to “stop shedding overnight.” Instead, we focus on creating favourable conditions for the hair follicles to return to a normal growth cycle.
This may include:
- Correcting identified nutritional deficiencies
- Improving protein and overall nutritional intake
- Managing scalp inflammation
- Maintaining regular scalp cleansing
- Managing underlying medical triggers with the appropriate healthcare provider
- Reducing ongoing physical or psychological stress where possible
- Monitoring regrowth over time
8. Low-Level Laser Therapy
For appropriate patients, low-level laser therapy (LLLT) may be discussed as part of a broader hair-growth program.
Freedom Clinic Toronto commonly uses devices such as the HairMax LaserBand 82 or Lumina, generally several times per week.
Laser therapy is not a treatment for the underlying medical trigger that caused telogen effluvium, but it may be considered as a non-invasive supportive therapy, particularly when there is delayed recovery or concurrent pattern hair loss.
9. When Hair-Growth Medication May Be Appropriate
Not every patient with telogen effluvium requires hair-growth medication.
This is an important distinction.
If microscopic examination shows healthy follicular density and no significant miniaturization, the appropriate approach may simply be to identify the trigger, optimize scalp health and monitor recovery.
However, if examination reveals underlying androgenetic alopecia, prolonged follicular dormancy or reduced hair-shaft diameter, additional treatment options may be discussed.
Depending on the patient’s diagnosis, medical history and suitability, these could include topical or oral therapies prescribed or supervised by an appropriate healthcare professional.
At Freedom Clinic Toronto, treatment recommendations are based on the actual diagnosis rather than automatically prescribing the same hair-loss treatment to every patient who is shedding.
10. TrichoBoost for Appropriate Hair-Loss Cases
For some patients with underlying androgenetic alopecia, a compounded topical treatment such as TrichoBoost may be considered.
TrichoBoost contains a combination of hair-growth and anti-androgen ingredients and is intended primarily for appropriate pattern hair-loss cases rather than uncomplicated acute telogen effluvium.
This distinction is important.
If a patient is experiencing temporary telogen effluvium but has no underlying miniaturization, we may not recommend aggressive long-term hair-loss treatment.
If telogen effluvium has exposed previously unnoticed pattern thinning, treatment of both conditions may be appropriate.
11. PRP and Regenerative Treatments
Patients experiencing severe shedding often ask whether they should immediately begin PRP or exosome treatments.
These therapies are not automatically required for every case of telogen effluvium.
At Freedom Clinic Toronto, treatments such as PRP and regenerative scalp therapies are considered based on the condition of the follicles, the duration of hair loss and whether there is an additional diagnosis such as androgenetic alopecia.
A treatment should have a reason for being recommended.
Our objective is not simply to perform more treatments — it is to determine which treatments, if any, make sense for the individual patient.
12. Monitoring Regrowth Is Just as Important as Monitoring Shedding
Patients naturally focus on the hairs that are falling out.
From a trichology perspective, however, we also want to know what is growing back.
During follow-up assessments, we may monitor:
- New short regrowing hairs
- Changes in hair-shaft thickness
- Changes in overall density
- Reduction in shedding
- Follicular activity
- Scalp inflammation
- Changes in miniaturization
- Progress in specific areas of the scalp
Clinical photographs and microscopic comparisons can help provide a more objective picture of improvement.
This is especially useful because hair recovery happens slowly and patients may not notice gradual changes from day to day.
Our Goal Is to Find the Cause — Not Simply Label It “Stress”
Stress can certainly trigger telogen effluvium, but not every case of sudden hair loss should automatically be blamed on stress.

- Is this true telogen effluvium?
- What may have triggered it?
- Is the shedding still active?
- Is healthy regrowth occurring?
- Is there underlying androgenetic alopecia?

- Does the patient actually require treatment, or do the follicles primarily need time to recover?
These questions allow us to create a more individualized plan rather than treating every shedding patient in exactly the same way.
